<p>Gout is a rheumatic disease resulting from deposition of <strong>uric acid crystals</strong> in tissues and fluids within the body. This process is caused by an overproduction or under excretion of uric acid.</p>
<p>Acute gout will typically manifest itself as an acutely red, hot, and swollen joint with excruciating pain. <p>When gout is clinically manifest, however, acute treatment of gouty  arthritis should be followed by determination of the cause of  hyperuricemia, and long-term treatment to lower the uric acid level is  usually necessary.</p>
<p>In addition to reduced consumption of  purine-rich foods such as offal and seafood, patients should also limit  their consumption of fructose-containing drinks as these reduce the excretion of uric acid.</p>
<p>The diet should  be rich in milk and skimmed milk products and in vegetable protein.  An important element is limited consumption of alcohol: There should be  at least three alcohol-free days a week.</p>
<p>Beer should be avoided because of its high purine content, but a glass of wine is regarded as harmless in gout.</p>
<p>Careful weight loss of less than 1 kg/month with light physical  exercise is desirable;  more rapid loss of weight could lead to ketoacidosis, provoking gout  attacks.</p>
<p>Patients with a history of kidney stones are recommended to  drink more than 2 L/d.</p>
<p>These changes in life habits,  however, usually make only a moderate contribution to urate reduction.</p>
<p>A consistently purine-poor diet, for example, may be expected to result in a 10% to 15% reduction in serum urate concentration.</p>
<p>Given that for many gout patients the serum urate concentration measured before the start of treatment is 480 µmol/L (8 mg/dL), such a  reduction would result in a value of 400 µmol/L (6.7 mg/dL)—still above the required target range for gout patients.</p>
<p>As a general measure, the  patient should be given advice on possible changes of life habits that could lead to an improvement in his or her overall metabolic profile.</p>

			<content:encoded><![CDATA[<p></p><p>Looking at this causation of arthritis we can see that there must be two  chief forms in which it will be met with; one of acute arthritis, in  which the uric acid is driven quickly out  of the blood into numerous joints, and a more or less decided rise of  temperature results, partly owing to the complete clearance of uric acid from the blood and consequent rapid  combustion, and partly also to the local irritation in numerous parts of  the body.</p>
<p>This is known as acute arthritis, and in certain conditions  has been called acute rheumatism, and in certain other conditions acute gout.</p>
<p>I have long preferred merely to call it acute  arthritis due to uric acid.</p>
<p>Here we have  an acute trouble corresponding with very marked signs of the absence of uric acid from the blood, and the treatment which will relieve this condition is  to bring the uric acid back into the  blood as quickly as possible by means of solvents.</p>
<p>There is thus a law  that all local precipitation diseases are relieved by solvents, and that  all collaemic diseases are relieved by precipitants; in other words, by  <em><strong>those things which clear the uric acid out  of the blood and drive it into the fibrous tissues</strong></em> ; these being but  two sides of the same process.</p>

<p>But there is a second form of arthritis  associated with different conditions. The capillary reflux is not quick;  it may even be decidedly slow, and there are marked signs of debility  and anaemia; these demonstrating the presence of more or less chronic  excess of uric acid in the blood.</p>
<p>The uric acid troubles in the fibrous  tissues of the heart commonly called endocarditis and pericarditis and  ascribed to the unknown factor rheumatism, have really an identical  causation with the uric acid irritation  in the fibrous tissues of the joints which we have just been  considering.</p>
<p>Excess of uric acid in the  blood causes, as we have seen, a very considerable addition to the labors of the heart in driving the blood forward through the  capillaries into the veins.</p>
<p>When, under this increased strain, the  fibrous tissues in the valves of the heart become seats of diminished  alkalinity, uric acid is precipitated  into them in exactly the same way as into the fibrous tissues of the joints, and this is the reason  why joint troubles and heart troubles are so very commonly met with  together.</p>
<p>In the valves of the heart also we get first a small  irritation, then a small precipitation of urate, and in succession a  further extent of irritation and a further precipitation of urate, till a  comparatively large tract on the surface of any valve is involved in  the urate irritation.</p>
<p>This, I may mention in passing, practically always  affects the valves of the heart which are most exposed to pressure and  strain, just as outside the heart uric acid is  always precipitated on all those joint structures most in use or most  subject at the particular time to pressure, injury, or strain.</p>
<p>Once a  lesion has been produced on the valves of the heart, this lesion, like  that in any of the joints, may act as a uric  acid filter, attracting more and more uric  acid to itself for days, weeks, months and even years. People  are much too apt to imagine that an attack of endocarditis having once originated and been treated by solvents is over and done  with for ever.</p>
<p>This is very rarely the case, for if solvents are not  given for a sufficient length of time to extract all the uric acid from the damaged tissue, further  quantities of uric acid are certain to  find their way there, with the hourly and daily fluctuations in the  amount of uric acid and its solubility in  the blood and tissue fluids.</p>
<p>In this way it comes about that what was  at first a microscopic nodule of irritation on a valve, becomes  gradually a widespread roughened surface with more or less hypertrophy  of the fibrous tissues, and this irritation continued and kept up for  weeks, months and years, eventually ends in a contracted or even  calcareous mass of fibrous tissue, such as we find in the last stage of  what is known as the &#8221; button-hole mitral valve.&#8221;</p>
<p>What we learn from  this natural history of endocarditis is, I think, that the only safety  from its continued progression lies in the complete removal of the  original focus of uric acid precipitation, and in  the careful guarding of that focus from further precipitation in the  future by means of solvents and the diet which excludes excess of uric acid from the body.</p>

<p><em>Stigmata Maidis</em></p>
<p><em>Stigmata Maidis </em>possesses great virtue in irritable conditions of the urinary organs and is said to be the only known remedy capable of cleansing the kidneys from the dangerous deposits of brick dust in the urine and removing the condition which is the cause of it</p>
<p><em>&#8220;Stigmata Maidis </em>is indicated in cystic irritation, due to phosphatic and uric acid concretions. In these cases the urine is usually scant and of a strong odor. The remedy not only re­lieves the bladder and urethral irritation, but tends also to prevent the formation of gravel and calculi. It is an important and favorite remedy in the treatment of urinal disorders of the aged, especially where the urine is strong and scant, and throws a heavy sediment. <em>Stigmata </em>should always be thought of in in­flammatory conditions of the urethra, bladder, and kidneys, where it is evident that the inflammatory trouble is due to the presence of urinary concretions.&#8221;—<em>Dr. Neiderkorn</em>—<em>Lloyd&#8217;s.</em></p>
<p>Where there is a tendency to the formation of gravel, or where it is known to exist, give:</p>
<p>Tinct. <em>Stigmata Maidis </em>1 oz.</p>
<p>Tinct. <em>Triticum 1/<sub>2</sub> </em>oz.</p>
<p>Dose 10 to 60 drops in hot water as required. Dose of <em>Stigmata Maidis </em>in other conditions, 10 to 60 drops.</p>
<p>Buchu</p>
<p>Buchu leaves and oil of buchu were used by the indigenous people of   South Africa for hundreds of years. The medicinal use of buchu was taken   up by the early Dutch settlers and later introduced into the   pharmaceutical industry in the UK. Buchu preparations are now used as a   diuretic and for a wide range of conditions including stomach aches,   rheumatism, bladder and kidney infections and coughs and colds.</p>
<p><em>Barosma Crenata</em></p>
<p>Buchu is aromatic and a stimulant, a diuretic and a dia­phoretic. It is given in dyspepsia for a palliative effect, but is usually prescribed in chronic inflammation of the bladder, irri­tation of the membrane of the urethra, uric-acid gravel, diabetes in its first stage and in incontinence of the urine. Buchu is best combined with other agents harsher in their activity.</p>

Purines are found in some foods and drinks, such as liver,  anchovies, mackerel, dried beans and peas, beer, and wine. Purines are  also a part of normal body substances, such as DNA.</p>
<p>Most uric acid dissolves in blood and travels to the kidneys, where  it passes out in urine. If your body produces too much uric acid or  doesn&#8217;t remove enough of it, you may get sick and need <a title="Uric Acid Treatment" href="http://uricacidtreatment.com/">Uric Acid Treatment</a>. High levels of uric acid  in the body is called hyperuricemia.</p>
<p>This test checks to see how much uric acid you have in your urine.</p>
<div>
<p>Your doctor may tell you to stop taking any drugs that may affect the  test results. For example, high levels of vitamin C and dyes used  during certain x-rays may cause incorrect results.</p>
<p>Drugs that can interfere with test results include:</p>
<ul>
<li>Alcohol</li>
<li>Allopurinol</li>
<li>Anti-inflammatory drugs (such as ibuprofen)</li>
<li>Salicylates (including aspirin)</li>
<li>Thiazide diuretics</li>
<li>Probenecid</li>
</ul>
<p>This list may not be all-inclusive.</p>
<p>The test involves only normal urination, and there is no discomfort.</p>
<p>This test may be done to diagnose the cause of kidney stones. It may  also be used to monitor people with gout,  since many of these patients develop uric acid kidney stones and<a title="uric acid arthritis" href="http://uricacidtreatment.com/acute-arthritis-from-uric-acid/"> uric acid arthritis</a>.</p>

<p>Normal values range from 250 to 750 milligrams per 24 hours.</p>

<p>Higher than normal uric acid levels in the urine may be due to:</p>
<ul>
<li>Cancers that have spread (metastasized)</li>
<li>Disorders that affect the bone marrow or certain white blood cells</li>

<li> Rhabdomyolysis</li>
<li> Lesch-Nyhan syndrome</li>
<li>Fanconi syndrome</li>
</ul>

<p><em>Triticum repens</em></p>
<p>The <em>gravel </em>remedy.    A stimulating demulcent,  acting chiefly upon the kidneys, bladder, and urinary passages.</p>
<p>Medicinally, <em>Triticum </em>is classed as a mild, stimulating demulcent, acting chiefly upon the kidneys and bladder and urinary passages. It is soothing in cases of irritation of the kidneys, bladder and urethra, and is especially valuable in gravel, many experienced physicians claiming that its timely use has dissolved small calculi. It is indicated by catarrhal and purulent discharges from the urethra, in purulent cystitis, incipient nephritis, stangury, enlarged prostate, when due to chronic gonorrhoea, gout, jaundice, and rheumatism. In in­continence of the urine with intense burning sensation and constant desire to urinate, it is splendid in its results. Dose 5 to 30 drops. To help calculi to pass, the maximum dose of 40 to 60 drops in hot water should be given.</p>
<p>In chronic gonorrhoea, give:</p>
<p>Tinct. <em>Triticum </em>1 oz.</p>
<p>Tinct. <em>Hydrastis </em>1 oz.</p>
<p>Dose 10 to 30 drops four times a day in plenty of water.</p>
<p>Enlarged prostate and jaundice, give:</p>
<p>Tinct. <em>Triticum </em>1 oz.</p>
<p>Tinct. <em>Chionanthus </em>1 oz.</p>
<p>Dose 5 to 20 drops, 4 times a day in plenty of water.</p>
<p>In rheumatism with prostatic trouble, give:</p>
<p>Tinct. <em>Triticum, </em>5 to 10 drops in an infusion of Mother-wort.    Repeat three or four times a day.</p>
